The Personal Information Protection Act 2004 came into effect on the 5 September 2005 and regulates the way in which public sector agencies deal with personal information collected, maintained, used and disclosed in the course of delivering services.
Personal information includes:
- information or opinion in any recorded format, including photographs and sound and video recordings, about an individual that readily identifies that individual
- sensitive information and
- health information.
How can I access my personal information?
From 1 July 2010, applications to access personal information are made under the Personal Information Protection Act 2004 (the Act). We have prepared an Application for Personal Information form to help you apply to access your personal information.

What happens after you lodge an Application for Personal Information form
The operational unit will contact you promptly after receiving your application. They may need to ask you to provide further information to help them process your application. If you have not completed all details on the Application for Information form, or have not provided appropriate evidence of your identity, the operational unit may be unable to process your application.
Charges for information
We will advise you of any charges for accessing your information. Information may be withheld until all charges have been paid.
